Main Goal:

Development of a technological collaborative and predictive solution based on applications, smart devices and decision support systems for the different providers involved in the chain of care in the home hospitalization.

Description:

The project will permit to test a methodology of care, patient-centered and proactive, to promote an integrated care (social and health) on the home hospitalization with the aid of the information and communication technologies. Through this technological platform the aim is on the enhancement of the patient attention by sharing information between all actors, allowing the coordination of tasks, with the availability of decission support systems based on the generated information, integrated with the already existing information systems and looking for the optimization of resources.

Reasons for collaboration between Health and Social Systems, enhacement of the integrated care of the patients, equity in the access to health resources, sustainability of the Welfare Systems and the pursuit of excellence in the quality of the services offered have inspired SALUD-Sector Barbastro in the design of this project. Involving the social partners in the provision of integrated care during home hospitalization.
